OREANDA-NEWS.  Robert Miller was sick. So sick, in fact, that he was unable to make the short trip to his primary care physician’s office.

“When I first met Robert, he was confined to his bedroom in the back of his house. He had a hard time walking, couldn’t move his neck — he wasn’t able to leave his house much to get the medical care he needed,” said Kristine Woolley, Kaiser Permanente Colorado physician assistant.

Woolley is a part of the Complex Care Home Rounding (CCHR) program at Kaiser Permanente. It’s a physician-referral only program, available to all Kaiser Permanente members including Medicare members. It employs advanced practice nurses, physician assistants and physicians who visit home-bound Kaiser Permanente members in their place of residence.

Through multiple visits from Woolley and others on the CCHR team, Miller, a Kaiser Permanente Colorado Medicare member, is now doing well. He’s healthier, stronger and enjoys the care he’s receiving.

“I’m very happy with what I’ve been experiencing (with Kaiser Permanente). No question about it. The care I’m getting — you just can’t rate it. It’s superb,” said Miller.

Miller’s experience is just one of the many reasons why the Centers for Medicare & Medicaid Services rated Kaiser Permanente Colorado’s Medicare Senior Advantage plan 5 out of 5 stars — the highest overall rating for quality and service — for 2017. This is the seventh year in a row Kaiser Permanente Colorado has received this rating.

“We’re proud the Centers for Medicare & Medicaid Services recognized Kaiser Permanente with this 5-star rating. It represents the tireless efforts of our physicians, nurses and staff who help our members live the longest, healthiest lives they can,” said Roland Lyon, president of Kaiser Permanente Colorado. “As Coloradans age and their health needs rapidly evolve, we continue to look for ways to deliver on our promise of providing our members with the most convenient and highest-quality care possible — whether it’s in our facilities, over the phone, or in our members’ homes.”

Kaiser Permanente Colorado Medicare health plans have consistently received high ratings since CMS began awarding them in 2009. One reason is Kaiser Permanente’s integrated care model, which enables care teams and staff to serve members and patients using a coordinated, seamless approach.
